40 Under Forty Nominations


Please fill out the form below to submit your 40 Under Forty candidate packet. Deadline for submission has been extended. Please refer to your email notification.

Requirements for submission:

1. Personal statement - (1 page single space) - Discuss why you believe you should be recognized as a 40 Under Forty recipient; what personal or professional impact you've made to Southern University and/or the community in which you live; and what impact you plan to make as a result of receiving this award that will continue to shine a positive light on Southern University.

2.  Résumé

3. Copy of a state issued drivers license (for age verification)

4. Completion of Nominee questionnaire below. (Please hover over the questions in the questionnaire for instructions).

NOTE: All documents should be packaged as one PDF and uploaded below.

Nominees are encouraged to answer the questions below completely and concisely.  Evidence of claims made by nominee is not required but encouraged to give the judging panel a more robust picture of the nominee.
